We are out of network with all commercial health plans and we are not accepting assignment. Payment is due at the time of service. Upon request, we will provide a super bill which you may submit for reimbursement of covered services.  Cancelation Policy. We require a 48-hour business notice and notification to avoid fees. Monday or Tuesday appointments must be canceled by noon on the previous Friday. If you cancel with less notice, you will be required to pay up to the full appointment fee for late cancelation/no show penalty. We reserve the right to waive this policy at our sole discretion.
