We are out of network with all commercial health plans and we are not accepting assignment. Payment is due at the time of service. Upon request, we will provide a super bill which you may submit for reimbursement of covered services. Cancelation Policy. We require a 48-hour business notice and notification. Monday or Tuesday appointments must be canceled by noon on the previous Friday. If you cancel with less notice, you will be required to pay a $80 late cancelation/no show penalty fee. We reserve the right to waive this policy at our sole discretion.

You may schedule a same-day appointment up to 3 hours prior to a given time. Online scheduling is available up to 6 weeks out from the current date. I do get cancellations weekly: consider adding yourself to the waitlist for your preferred time.
